What do you think?

I think they need either better launchpad at Omelek to prevent fiery flashbacks to unprotected engine compartment, or shield the compartment a la Delta IV.

Is it possible that the launch pad failed in such a way that debris was created that impacted the rocket and caused the fuel leak?

Quite possible. In the launch video you can see a lot of debris in the air at liftoff, including one shack <img src="/images/icons/smile.gif" />

A lot different from KSC or Edwards, for sure. I agree about less problems if they had a more robust pad.
