Most people simultaneously in space, ever?

I was just thinking how with today's Soyuz launch there are now 12 human beings in orbit at the moment. And that triggered me to ask this question: What is the record for the most number of persons ever to be in space at the same time. And when and under what missions/circumstances did this occur?

13 people were in orbit simultaneously at least twice: first in 1995 (Mir/Soyuz TM-21/STS-67), then in 1998 (Mir/Soyuz TM-27/STS-89).
