Only goes to show Space X's mishap with starship? wasn't an isolated incident. Perfecting rocket technology isn't without risks as one works it out. In the early days of our space program, Apollo 13 wasn't the only disaster when the astronauts went to mix the oxygen tanks; I believe NASA tried pure oxygen during one of the Mercury launches that lead to disaster when they went to replicate it in Apollo 1.
