I agree with Calli on the causes of both accidents being traceable to NASA management culture. This would seem to indicate that the flawed, inherantly unsafe, shuttle design still has yet to have an accident that is not somehow traceable to management. A bonifide hardware failure resulting from...hardware failure!<br /><br />The type of failure that when investigated, the report would simply conclude that shuttle failure resulted from whatever.
