"The citations specify alleged failures by Scaled to adequately train its personnel in the use of substances such as nitrous oxide. It was a cold flow test of nitrous oxide that led to the explosion."
http://tinyurl.com/2ror9z

So what happens now? Will they be brought up on charges? People are still buying tickets.
People have said for years that the first fatal accident will shut down private spaceflight. I've said for years that it won't work that way. There simply is no mechanism to make that happen. There are however many mechanisms to keep things moving forward, insurance being the primary actor.

Fatal industrial accidents are nothing new. Criminal culpability is rare, and even when present, the organization typically moves forward anyway. There is zero indication of criminal negligence in this case AFAIK.
