I just came across this article which details some of the problems NASA had in developing the TPS for the Orbiters. Interesting reading (then again, I think drwayne's articles about pogo are interesting! <img src="/images/icons/tongue.gif" />)

Some of that was tough going for my simplistic CPU, but what an interesting read. They must have thought they had created a monster when tiles seemed to be coming off due to faults as quick as they were going on.<br /><br />I also found it really interesting how they arrived at the eventual implementation of the TPS by quite a convoluted route, compared to how they initially thought they would implement the system. "every time I see TPS ... I think of <i>Office Space</i>"<br /><br />Me too, I spent 5 years moving Test Preparation Sheets around Mission Control. Then I moved onto the world that uses Task Performance Sheets as the instrument to get anything done. I cracked up the first time that guy's smarmy boss hung around outside his cube and started ragging him about getting his TPS signed off.

the main character gets in repeated trouble for failure to properly use cover sheets on his TPS reports .... (yeah, it was off-topic and random)
