The ones I can remember are:-

No more than three EVA's (Gene Cernan found that this could not be changed for A 17)

If landing radar fails then a missionis aborted

Oops thats all i can rember of the top of my head

The rover could not go further than it would take for the crew to walk back in the event of failure(Pete Conrad thought this could be doubled as you move faster on the moon)

The 'Rule Book' for Apollo - if any such single book existed - would be an inch thick! There were rules concerning every stage of the mission, from pre-launch to post-landing.
(Of course, there would be thousands of penciled in addendums in the book, but that's another story.)
